The Plymouth State University Athletic Department, in conjunction with Athletic Training Services, requires that all prospective or current student-athletes complete health information, consent and emergency contact information forms prior to each academic year. These forms are protected by medical and personal confidentiality guidelines under the Family and Educational Right to Privacy Act (Buckley Amendment) and the Health Insurance Portability and Accountability Act (HIPAA). Information contained in these forms will be used by the athletic training staff and/or team physician(s) to provide the best possible healthcare for student-athletes during their participation in the athletic program at Plymouth State University. This information will not be shared with any other individuals or entities without the express written permission of the individual student-athlete.

"I chose to attend Plymouth State University to pursue a degree in the growing field of Criminal Justice and to have the chance to play a collegiate sport at a high level. Besides having a very good Criminal Justice program, PSU has excellent athletic teams and staff. Plymouth State’s relatively small size has given me the ability to pursue my degree while playing a sport. Being a part of the football team has allowed me to make a ton of new friends since first arriving on campus. In my short time here, I have learned excellent life skills on the field and in the classroom, which will all help me to succeed in my future."
